Agency Referrals in CLEO

Agency referral enables your case notes to be emailed to some (but not all) receiving services when referring patients. You can use agency referral to refer to

• Clevedon MIU
• Yate MIU
• South Bristol Community Hospital UTC
• Weston, BRI and Southmead ED
• Children’s ED

Instructions are provided in the short video about how to use agency referral. It is a quick and easy process, which results in your full case notes being emailed to the relevant service. You can use agency referral when the receiving service is closed. You can advise patients that the notes from your consultation have been emailed.

Referrals to other services should be made as per instructions on the relevant Toolkit page and/ or Remedy.

Please note that CLEO consultations should also be visible on Connecting Care within an hour of you closing the case records. This supports sharing of information with secondary care colleagues when arranging admissions eg to medicine/ surgery. Similarly, CLEO records transfer as a post-event message (‘PEM’) to EMIS in approximately the same timeframe.
