To ensure patients receive the appropriate care, it may be necessary for some patients to be referred from Severnside IUC to another service to continue their care. To ensure that we provide a robust single point of contact for the services our patients are referred on to, we ask that you do not send onward referrals for patients you consult in Severnside IUC from your personal nhs.mail account.
Below is an outline of how to store, send and delete onward referrals.
- Complete the relevant referral form and save it in the “patient forms” folder. The location of this for each Treatment Centre but it can be found here:

- Please save the referral form using the CLEO case number with patient initials as the file name.
- Once the referral form is saved in the “patient forms” folder, please attach and email the referral form from your own nhs.mail email account to the shift manager at severnside.referral@nhs.net.
- Please include the following information in your email to the shift manager:

- Patient demographics (name, DOB, NHS number)
- Speciality being referred to (e.g., Patient TIA referral Patient form).
- Please ensure that no referral forms containing patient details have been saved onto computer desktops.
- Once you have sent the form to the shift manager, please then delete it from the “patient forms” folder
- The shift manager will forward on the referral form to the relevant service.

If you do not have an nhs.mail email account, then please ask the Treatment Centre Host to email the referral form to the Shift Manager on your behalf.
