Respiratory hot clinic

NBT

The Respiratory Hot Clinic at NBT is an admission prevention clinic for patients threatening admission with a respiratory cause.

Referrals will be screened by a Specialist Respiratory Nurse or Respiratory Physician prior to accepting/declining.  Where patients cannot be seen, the referring health care professional will be informed and the responsibility of care taken back by them.

Referrals are made via email, however the

Resp Hot Clinic consultant can be contacted on 0117 414 0282 to discuss whether a case would be suitable as part of admission avoidance.

Other ways to contact:

  • Call medical SDEC and ask for the Resp Hot Clinic Consultant
  • Respiratory SpR available through switchboard

Once a referral is made, the patient will be seen within 24 hours on a same day/next day service. They will be seen by the Hot Clinic Facilitator and Respiratory physician in clinic and, if possible, be discharged back to the community within the same day. The service is supported by Respiratory Specialist Nurses who provide home support during the patient’s acute illness.

The service runs from Gate 12 on the Southmead Site from 08:00- 17:00 hrs. Referrals received where patient is able to arrive by 14.30am will be seen the same day, otherwise next day service.

Inclusion Criteria:

  • Emailed referral from a community health care professional for patient threatening admission with a respiratory cause
  • Patient must be available to be seen within 24 hours of referral
  • For exacerbations of COPD or asthma, we would expect that the patient had at least been given a trial of steroids/antibiotics first in primary care; i.e. in the last 2 weeks..

Exclusion Criteria:

  • Suspected lung cancer in well patient to be referred via ‘2-week wait, fast track’
  • Patients who cannot attend within 24 hours
  • Patients who are referred on a Friday and cannot be seen the same day will be put back in the care of the referring health care professional.  They can be advised to either admit or re-refer the next week if still concerned about the patient’s condition

Referral forms (word doc) should be emailed to FAXRespiratoryHotClinic@nbt.nhs.uk. Please make sure that you include list of medications, allergies and past medical history with every referral. Include [secure] as the first word on the “subject” line as this is not an nhs.net email. The service runs Monday to Friday excluding bank holidays. Please ensure the patient’s contact telephone number is included in the referral form. They will be contacted by phone to give them an appointment time.

BRI

Admission prevention for new or deteriorating conditions.

Exclusion criteria:

  1. Covid-19 disease
  2. Pleural effusion (refer to pleural clinic via eRS),
  3. Suspected cancer (two week wait referral via eRS)
  4. TB (TB Clinic via eRS)
  5. Chronic sub-acute disease that can wait for a routine referral.

Referrals should be made via eRS to the Respiratory HOT Clinic RAS using the Hot Clinic Referral Form (word doc).  All referrals will be clinically triaged and a suitable appointment offered.

Please ensure patient contact telephone number is included. They will contacted by phone to give an appointment time.
