Head Injury in Frail Patients

Updated on 13th May 2026

BNSSG have developed a framework to support the community management of frail patients (CFS 5 & above) on anticoagulation &/or antiplatelets (excluding aspirin monotherapy), who have fallen and sustained a head injury/suspected head injury.

You can access the guidance on Remedy here : Head Injuries In Frail Patients

The guidance is intended to support clinicians to practise pragmatic, patient-centred, evidence-based decision making when agreeing management decisions with patients and their families, regarding

  1. Conveyance to hospital for imaging and
  2. Ongoing treatment with anticoagulation/antiplatelets

This tool has been developed by a steering group of BNSSG clinicians with representation from GPs, SWASFT, and Consultants in Emergency and Geriatric Medicine.  It has been endorsed by the BNSSG Urgent and Emergency Care Operational Delivery and Urgent Care Interface groups.

Please email any feedback on this new guidance to brisdoc.governance@nhs.net
