NHS@home - Respiratory pathway

BNSSG NHS@Home provides clinical care for people who are acutely unwell in their own homes, via a ‘virtual ward’ service. The service enables people to get the care they need at home safely and conveniently, rather than being in hospital.

NHS@Home uses a mixture of digital monitoring on a virtual ward, telephone support and face to face visits from specialist teams. The specialist service has access to support from acute consultants.

There is now capacity under the NHS@Home respiratory pathway. Please use this service today to support safe and effective acute healthcare in the home setting.

The Respiratory Pathway includes adult patients with a respiratory illness (new or exacerbation of a chronic respiratory condition) who require clinical care to remain safe in their own home environment. This includes:

  • Exacerbation of COPD.
  • Exacerbation Asthma
  • Lower respiratory tract infection.
  • Mild pneumonia (CURB<2).

NHS@Home uses a mixture of digital monitoring on a virtual ward, telephone support and face to face visits from specialist teams. The specialist service has access to support from acute consultants. This can include bloods, nebulisers, and IV antibiotics if required.

Referrals can be made or discussed by contacting .0300 125 5001 , 7 days a week (8am -8pm).

Consider discussing borderline cases that may not strictly meet the criteria if it would lead to admission avoidance.
