- Add notes to both Rio and Cleo.
- In Cleo, select End Assessment and complete the End Assessment questions.
Assessment Only Non-Registered Clinicians
- Add notes to both Rio and Cleo.
- In Cleo, select End Assessment.
- Select ‘Yes’ for Registered Mental Health Sign Off.
- Answer the remaining End Assessment questions and select Complete.
- The case will land back in the CAS queue, with a sub classification indicating it requires Registered Sign Off.
Secondary Input for Physical Symptoms
This guidance is for BNSSG patients only.
Mandatory verbal conversation: IAP clinicians must have a verbal conversation with the SevernSide Clinical Coordinator to ensure they fully understand what is being requested and to minimise any unnecessary delays to patient care. If there is not a SevernSide Clinical Coordinator on shift: please ensure the BrisDoc Shift Manager is made aware of the referral.

- In Cleo, select Follow Up and then Further Urgent Care Follow Up. Add a time and priority for the case once it becomes active, then click Confirm.
- Select End Assessment and answer the usual questions.
- The case will close and a new case will open in the Urgent Follow Up queue for the Physical Health Clinical Coordinator to review.
- Please inform the Shift Manager, who will make the case active straight away, if required.
- What is/ are the physical health symptom/s?
- How long have you had this physical health symptom?
- Has it changed since it started? (ideally noting how it has changed)
- Have you ever had this physical health symptom before?
- Is it the same or different this time? (ideally noting how it is different)
- Is this physical health symptom stopping you from doing anything you can normally do?
- (ideally noting what it is stopping)
Any BSW patients needing support with physical health should be directed the most appropriate route (OGP, NHS111 or 999).

- Document clearly the reason for input from the Physical Health Clinician. Include:

Referral to Own GP for Next Day Follow Up
If the patient meets the Practice Liaison Service (PLS) criteria, you can select GP to Contact Patient on the End Assessment questions. Please ensure you have clearly documented what input is required by the patient’s own GP within your consultation.

Further Call Back / Attempted Call Unanswered by Patient

Deploying a Mental Health Response Vehicle (MHRV)
- Contact the MH CC to request its availability, via phone or MS Teams.
- If the MHRV is available and the deployment is agreed, add case notes to reflect the patient’s onward pathway and accepted referral to 999.
- Close the Cleo case, marked as referred to 999.
- The MHSD must open a new case on CAD and the management of the patient continues in line with MHRV operation.
- If further intervention is then required by SevernSide/MH CAS, a new case must be opened via the SevernSide Professional Line (0117 244 9283).
Red Flags for Physical Health
The aim of this quick guide is to give Call Handlers more confidence when making safety calls to patients and to help identify any major worsening symptoms.
We do not want you to become clinicians or start giving advice. We want you to be aware of some of the more severe ailments, what to look for and how to react.
When speaking to a patient, if they inform you of any of the symptoms below, advise the patient or relative to call 999 and escalate to the Clinical Navigator or Shift Manager immediately.
- Facial Weakness: Can the person smile? Has their face dropped on one side?
- Arm Weaknesss: Can they raise both arms above their head?
- Speech Problems: Can the person speak clearly & understand what you say? Does the patient have difficulty finding words or have slurring of their speech?
- Time: Act fast if any of the above are present.
Alert the Clinical Navigator or Shift Manager immediately.
- Is the pain central (crushing)?
- Any tingling or heaviness of jaw?
- Any tingling or heaviness of left arm?
- Does the patient look pale / sweaty / clammy?
- Does the patient appear breathless?
Alert the Clinical Navigator or Shift Manager immediately.
Severe and visible bleeding from any orifice.
Alert the Clinical Navigator or Shift Manager immediately.
Has the patient collapsed or are they unresponsive?
Alert the Clinical Navigator or the Shift Manager immediately.
A Physical Health Ambulance (999)
There may be some clinicians within the System CAS Service who are PaCCS trained and therefore might be able to request an ambulance electronically for BNSSG patients. Please discuss this with the Shift Manager if you feel this may be required.
Alternatively, please call the ambulance via 999. For BSW patients ambulances must always be accessed via calling 999.
