Handover from WDPL to IUC OOH

There are occasions when further input in the IUC OOH period can contribute to admission avoidance efforts for WDPL cases.

Examples include:

  • IUC OOH appointments to eg take / drop off urgent bloods ( eg PE pathway), or clinical review of patients at high risk of admission.
  • Clinical coordinator review of bloods taken to support admission avoidance or that could alter management in the OOH period

If OOH input is required, please ensure that the handover is clear for colleagues in relation to next steps. Key points to consider are as follows:

  •  indicate the plan at the top of your CLEO notes,
  • Ensure the correct contact details for the patient are on the CLEO records ( pls speak to the SM if you need advice on how to change this)
  • If any form of  F2F appointment is required, select the  F2F button Follow up  to request this ( eg blood test required/ F2F OOH appt/ Home visit request)
  • If review is required- select Follow up button followed by 'Further Urgent Care follow up required' selecting the time frame you would like the case followed up, for the case to be reviewed by the CC.

 

  • If any form of  F2F appointment is required, select the F2F button to request this ( eg blood test required/ F2F OOH appt/ Home visit request)
  • If review not requiring F2F appt is required- select FORWARD button to send the case to the CC advice queue.

 

Please also inform the WDPL shift manager of any cases forwarded to the OOH period, so they can highlight at the time of their handover.
