Once you've watched this video, please read this very important message from Dr Kathy Ryan regarding the use of EMIS.
To Amend an End of Life Chart in EMIS
- Find the most recent chart in documents in EMIS
- Click save as and save in the patient forms folder - more information about how to find this folder at different treatment centres can be found here
- Open the folder where the EOL chart was saved and open the document
- The chart can now be edited and saved
Once filled out please email to the SevernSide referral email account severnside.referral@nhs.net and instant message the shift manager on shift to notify them of your having done this. They will then forward your email/referral on your behalf to sirona.psd@nhs.net. This ensures that any replies or queries are able to be dealt with by SevernSide even if you have gone off shift by the time they come in.
