Patient Satisfaction Questionnaires for GP Appraisal

We are keen to support colleagues working in SevernSide to meet the requirements of appraisal and revalidation.  If you need to complete your PSQ, please follow this process:

  1. Seek verbal consent to obtain feedback from the patient during the consultation. Document their consent on CLEO
  2. If you use Fourteen Fish, send the PSQ link to the patient using CLEO. We suggest the following template for this text message ‘Thank you for agreeing to provide anonymised feedback on your consultation today.  I would be grateful if you could click on the link below to complete this.’ You copy and paste this template into your template list on AccuRx account.
  3. If you use Clarity, you will need to ask the patient for their email address and type this straight into the box in Clarity (please don't write the email address down).  More information about how to do this in Clarity can be found here.
