What is it?
GP Connect is a functionality within Adastra that enables OOH clinicians to add patients who are vulnerable with ongoing clinical needs directly to GP surgeries worklists for urgent follow up.
When should I use it?
GP Connect is for use in two instances:
- Patients who need urgent follow up with their practice, where you have concerns that they may not or cannot make contact with their own GP practice.
- Abnormal pathology results where we have not been able to contact the patient.
The PLS process is in place for:
- Death notification
Please only use the GP Connect function in the two scenarios outlined above.
GP Connect should not be used for clinical follow up when the patient or their relative/carer are able to contact the GP practice themselves.
How will it work?
Once clinical notes and coding have been completed within a case, there is a new button alongside finish, forward etc. This is ‘Book External Appt (GP Connect)”.
Selecting this will start the process for referral to the worklist. This will then walk you through the steps, ultimately adding the patient to the worklist.
It will appear that you are booking a timed face to face appointment – this is not the case so please advise the patient that the surgery will contact them via phone to follow up the next working day. It is not in person, and not a timed appointment.
Once this part of the process is done you will need to select the ‘finish’ button and close the case as usual.
For GP connect cases, you must click ‘no’ to the practice liaison question. The PEM will be sent as for all cases.
What if it doesn’t work?
- Check to see if you are logged into Adastra with your Smart Card.
- If no slots are showing, please refer via the PLS route and flag the case to the Shift Manager.
We are aware of one surgery (Kennedy Way) has not sign up to the GP Connect functionality. Continue to use the current PLS route for this practice at all times.
