Admission Reflection Tool (ART): Introduction

The aim of the tool is to improve confident shared decision-making, specifically relating to emergency admissions for frail people. The tool has been designed to encourage ’in the moment’ reflection on a range of pertinent factors. It is not designed to guide a decision in either direction.
