Scabies

Scabies – Quick reference for GPs (including OOH)

Key points: Contagious mite infestation causing intense nocturnal pruritus. Look for small erythematous papules, burrows and excoriations at classic sites. Spread is mainly via prolonged skin-to-skin contact. Prompt diagnosis and treatment relieve symptoms, prevent secondary infection and limit community transmission.

What it is

  • Human-specific mite (Sarcoptes scabiei var. hominis) burrows into the epidermis to lay eggs.

  • The host develops a delayed hypersensitivity reaction, driving the itch.

Clinical features

  • Symptoms: Intense pruritus, worse at night.

  • Signs: Small erythematous papules, burrows and excoriations.

  • Typical distribution: Interdigital spaces, wrists, axillae, waistline and genital areas.

Transmission and settings

  • Primarily via prolonged skin-to-skin contact.

  • Outbreaks occur in households, care homes, childcare centres and other close-contact environments.

Why early action matters

  • Symptom relief for the patient.

  • Prevention of complications, especially secondary bacterial infection.

  • Public health control to reduce spread in the community.
