ReSPECT forms / DNAR forms in IUC

ReSPECT

ReSPECT forms a unified approach to the recording of emergency treatment plans and DNAR decisions. ReSPECT stands for Recommended Summary Plan for Emergency Care and Treatment. It is about creating a personalised recommendation for someone’s clinical care in emergency situations where they are not able to make decisions or express their wishes. It will also record a Do Not Attempt Cardiopulmonary Resuscitation (DNAR) decision if one has been made. However, ReSPECT is about much wider care preferences and treatment ceilings and is equally applicable to patients for whom resuscitation is appropriate.

In IUC, please be proactive about ReSPECT discussions, decisions and documentation if the patient especially if the patient is rapidly deteriorating or likely in the last days of life. It is very important that a copy of the ReSPECT form ends up in the patient's residence, and a copy is sent to the practice as well.

You can complete a ReSPECT form digitally or on paper. Whichever you do, please ensure that you document completion of the form in CLEO, along with the decisions you have agreed with the patient/ family about the plans for their care, and whether or not you have agreed a DNAR decision

Completing the electronic PDF or Word version of the ReSPECT form 

  • Electronic ReSPECT forms should be saved in the Patient Forms folder on the O drive. Further details on how to locate this can be found here
  • Then please attach and email the referral form from your own nhs.mail account to the shift manager at severnside.referral@nhs.net - please include the patient NHS number and CLEOnumber in your email.
  • Please ask the Shift Manager to email this to the patient's own GP and Sirona's SPA (so that it is uploaded to both practice and Sirona EMIS platforms) and brisdoc.careplan@nhs.net (so the content can be uploaded as a special patient note in CLEO).
  • Please ensure you also let the Shift Manager know about the form waiting to be sent either in person (if in the same building) or via screen message/ Cleo Chat.

    • Practice email addresses can be obtained from NHS Service Finder but, if not available, this can be obtained by telephoning the practice back door number in hours.
    • If the email address is not available on NHS Service Finder during the out of hours period, we suggest using PLS so the PLS team can obtain the practice email address and send the form the next working day.
    • The form will need to be printed so there is a copy in the patient's home. If Sirona colleagues will be visiting the patient after the OOH involvement, then they may well be able to print and leave it in the home, but please ensure that request is handed over and the family know this is needed.

Completing a paper ReSPECT form

  • Paper forms are stocked at the Treatment Centres and in the visiting cars
  • Paper ReSPECT forms should be left with the patient at their home. The main downside of handwritten forms is that the form is not easily transferred to Sirona or the patient's own GP. As before, ensure the content is detailed in the CLEOrecords, including that a handwritten version is at the home
