The full list of notifiable diseases can be found on the gov.uk website here.
Do not wait for laboratory confirmation of the disease. By law, you must report any suspicion of a notifiable disease.
Notification process
Communicable diseases which may require urgent UKHSA involvement (eg meningitis), should be communicated by telephone to the on-call UKHSA person.
Medical practitioners must complete and send off the notification form to UKHSA. This form requires the practitioner to enter an email address and telephone number, please use severnside.referral@nhs.net and the professional line telephone number (0117 244 9283). Please note, the patient's name needs to be entered in to the form in lower case
UKHSA have asked that all urgent notifications during the out of hours period are telephoned through to them (not faxed). The out of hours telephone number for urgent referrals is the same as the in hours number: 0300 303 8162.
