Charlton Farm Childrens’ Hospice is in Wraxall, and provides children’s hospice and professional family support services to children with life-limiting illness. In addition to specialist palliative care, Charlton Farm provides respite for familiies, a sibling service, emergency support, end of life care and a bereavement service.

Many patients and their parents/ relatives/ carers are at the Hospice for respite care and support, so Primary Care problems can and do arise. BrisDoc provides primary care support for inpatients and their relatives at Charlton Farm, via the BrisDoc Professional Line.
Staff can contact BrisDoc during the OOH period to seek advice or assessment for patients or their carers. If clinically necessary, this may include visiting a patient at the Hospice. Please not that ALL palliative care or symptom control needs for patients who are end of life are managed by the on call clinical team at the Hospice, and should NOT be assessed or managed by BrisDoc Primary Care clinicians.
