This opioid dose conversion table can be used if patients need to be converted to an alternative opioid, or to change the route of administration of an opioid. Injections should be prescribed subcutaneously, not intramuscularly (painful) and intravenous administration is rarely necessary for managing cancer pain. Ratios are approximate; consider reducing the dose of new opioid in the presence of existing opioid toxicity.
